Many Planned Parenthood affiliates provide informed-consent gender-affirming hormone therapy alongside sexual and reproductive health care, on a sliding scale and with Medicaid. Most affiliates see adults 18 and older; a few see 16- and 17-year-olds with parental consent and a readiness letter. Many start hormone care by telehealth. Self-pay initial consults typically run about $120 to $250 before sliding-scale discounts.

Paying for care

Medicaid (Michigan Medicaid (including the Healthy Michigan Plan))[1][2]

The Medicaid Provider Manual (Practitioner chapter, section 3.13) says Medicaid "covers medically necessary gender affirmation/confirming medical, surgical, and pharmacologic treatments and procedures" for people diagnosed with gender dysphoria, and that these are not elective or cosmetic when medically necessary. Providers must follow current WPATH and Endocrine Society guidelines.

Who qualifies: the Healthy Michigan Plan covers Michigan residents 19 to 64 with income at or below 133% of the federal poverty level who do not qualify for Medicare or other Medicaid; other Medicaid programs cover children, pregnant people, and people with disabilities. Work requirements begin January 1, 2027 under federal law, so watch for notices from MDHHS. Apply online at MI Bridges or at your local MDHHS office.

Private insurance[5][4][3]

The Movement Advancement Project reports that Michigan bars health insurers from excluding transgender-related care, and the Elliott-Larsen Civil Rights Act (amended in 2023) prohibits discrimination based on gender identity or expression in public accommodations and services. If a plan denies care, appeal with the plan first, then file a complaint with the Department of Insurance and Financial Services (DIFS). Self-funded employer plans are federally regulated; the Trans Health Project has appeal templates.

No state restriction. Planned Parenthood of Michigan and several community clinics prescribe hormones on an informed consent basis, with no therapist letter, and telehealth prescribing is legal.

If you have no insurance and little money, in rough order:

  1. Apply for Medicaid / Healthy Michigan Plan at MI Bridges. It covers hormones, labs, and surgery when medically necessary.
  2. Go to a community health center (FQHC) or LGBTQ+ clinic such as Corktown Health, which take Medicaid and offer financial assistance. Find others at findahealthcenter.hrsa.gov.
  3. Planned Parenthood of Michigan offers hormone therapy at health centers statewide and by telehealth for ages 18 and up.
  4. Point of Pride's HRT Access Fund pays for telehealth hormone care for people who qualify.

Rules for under 18Legal. Michigan has no law restricting gender-affirming care for minors; care follows clinical guidelines and requires parental consent.

Legal. Michigan has no law restricting gender-affirming care for minors; care follows clinical guidelines and requires parental consent. Bills to ban it were introduced in 2025 but none has passed either chamber. Michigan has no shield law, so it offers no special state protection against other states' subpoenas or prosecutions. Ruth Ellis Center's Health & Wellness Centers (with Henry Ford Health) provide hormone therapy for young people ages 13 to 30 at no out-of-pocket cost.
