Adults (19 and older) can get hormone therapy and surgery in Nebraska without state restrictions. For anyone under 19, surgery is banned and puberty blockers and hormones are allowed only under strict state regulations, which most providers do not offer. There is no state shield law and no state protection against insurance exclusions.

If you are under 18, or a parent

The Let Them Grow Act (Neb. Rev. Stat. 71-7301 to 71-7307, from LB 574, in effect since October 1, 2023) covers everyone under 19:

  • Surgery is banned. Health care practitioners may not perform gender-altering surgery on anyone under 19.
  • Puberty blockers and hormones are allowed only if given in compliance with regulations from the state's chief medical officer, which set minimum therapy hours, informed-consent and record-keeping rules, and a waiting period. Treatment that began before October 1, 2023 may continue.
  • Penalties: a violation is "unprofessional conduct" that can lead to discipline of the provider's license. A patient or parent can sue within two years of discovering harm.
  • No state funds may pay for procedures that violate the act.
  • Exceptions exist for intersex conditions and for treating complications.

In practice, few Nebraska providers offer hormones to minors. Nebraska Medicine says it treats minors for behavioral health only. Nebraska has no shield law. Families often seek care in neighboring states; check that state's laws first.

Community health centers near you

These federally funded centers must see you whether or not you have insurance, and charge based on income. Many, but not all, prescribe hormone therapy. Call and ask for "gender-affirming care" or "hormone therapy."

Hormones by telehealth

These services prescribe hormones through video visits for people in this state. Point of Pride pays for a year and a half of care with several of them each spring.

  • Plume

    PaidNo Medicaid18+

Trans-led telehealth service providing informed-consent hormone therapy, lab orders, prescriptions, and letters for surgery and documents, by app and video visit. Self-pay only since January 2026. A Point of Pride HRT Access Fund partner.

About $99 per month ($90 in NY and NJ), which includes visits, messaging, prescriptions, and document letters. Labs and medications are billed separately and are often cheap with GoodRx or at a community pharmacy. Free for 18 months if you win a Point of Pride HRT Access Fund grant.

  • Planned Parenthood

    Sliding scaleTakes insurance

Many Planned Parenthood affiliates provide informed-consent gender-affirming hormone therapy alongside sexual and reproductive health care, on a sliding scale and with Medicaid. Most affiliates see adults 18 and older; a few see 16- and 17-year-olds with parental consent and a readiness letter. Many start hormone care by telehealth. Self-pay initial consults typically run about $120 to $250 before sliding-scale discounts.

Paying for care

Medicaid (Nebraska Medicaid (Heritage Health))[11][3]

No DHHS page states whether Medicaid covers gender-affirming hormones or surgery for adults. State funds may not pay for care to people under 19 that violates the Let Them Grow Act. Ask your Heritage Health plan for its written policy and request prior authorization in writing; if denied, appeal and contact the Trans Health Project for help.

Private insurance[12]

Nebraska has no law barring private health plans from excluding gender-affirming care. Read your plan's exclusions. You can file a complaint with the Nebraska Department of Insurance about a denial of a covered benefit; self-funded employer plans are regulated federally.

The rules in detail

Rules for adultsNebraska law does not restrict gender-affirming care for adults.

Nebraska law does not restrict gender-affirming care for adults. Note that in Nebraska the age of majority is 19, so the minors' law covers 18-year-olds too.

If you have little money, in rough order of what to try:

  1. Apply for Nebraska Medicaid (Heritage Health). Adults 19 to 64 with low income can qualify. Coverage of gender-affirming care is not clearly published, so ask your health plan before starting.
  2. Go to a community health center (FQHC) such as OneWorld or Charles Drew in Omaha. They charge on a sliding scale; ask whether a provider there prescribes hormones.
  3. Planned Parenthood North Central States lists gender-affirming care at its Omaha and Lincoln health centers and runs a Nebraska telehealth center.
  4. Nebraska Medicine's Gender Care Clinic in Omaha (UNMC) sees adults 19+ for primary and specialty gender care and surgery and can help with documents for gender marker changes.
  5. Telehealth: Plume serves Nebraska. Point of Pride's HRT Access Fund pays for telehealth hormone care for some applicants each year.
