You can update the name on a Missouri license or ID at any license office with a certified court order. Changing the sex marker is much harder: the Department of Revenue no longer uses its old gender-change form and does not publish what it now accepts. Advocates report it requires a court order or a surgeon's letter. There is no X option.

How it works
Missouri licenses and nondriver IDs are issued by the Department of Revenue (DOR) through local license offices (many run by contract agents). Any change to your name or sex marker is processed as a duplicate card, in person.
Name: bring a certified copy of your name change judgment plus proof of identity, Social Security number, and Missouri residency. This is routine.
Sex marker: for years DOR accepted a physician's statement form. According to Advocates for Trans Equality, in 2024 DOR stopped accepting that form, and updates now require an in-person application with a court order or a letter from a surgeon. DOR's own website only says "additional documents may be required ... to request a change to the gender" and does not list them. Only M and F are available.
If you were born in Missouri and have already amended your birth certificate, bring it; an amended birth certificate is one of DOR's listed name change documents, but whether it is enough on its own for the sex marker is not stated.
Steps
Bring:
- A certified copy of your name change judgment (or amended birth certificate).
- Proof of identity, your Social Security number, and Missouri residency (see DOR's document list).
- Update your name with Social Security first; DOR checks your Social Security record.
If you are also getting a REAL ID-compliant card for the first time, bring the extra REAL ID documents.
DOR does not publish its current requirement. Based on A4TE's report, bring one of these:
- A certified court order directing or recognizing the change of sex, or
- A letter from your surgeon confirming gender-affirming surgery.
If you have an amended Missouri birth certificate, bring that too. Call the license office before you go, because practice may differ between offices. There is no X marker.
A new signature is captured and a new photo may be taken. A duplicate Class F (regular) license costs $16.50 (3-year) or $25.50 (6-year); a nondriver ID costs $24. The license office adds a processing fee. If your card expires within six months, renew instead.

After you finishKeep the certified documents you used.
Keep the certified documents you used. Your old card is invalid once the duplicate is issued. If DOR refuses the sex marker change, ask for the refusal and the rule it is based on in writing, and contact the ACLU of Missouri or PROMO.
Tips2 tips from people who have done this
- Change your name at Social Security at least a day or two before going to the license office so the records match.
- If your card is within six months of expiring, you can renew early and make the change at the same time, avoiding a separate duplicate fee.
