How it works
The Nebraska DMV changes the sex on a license, permit, or state ID when you bring its Certification of Sex Reassignment form, signed by a doctor of medicine or osteopathy, a physician assistant, an advanced practice nurse, or a doctor of chiropractic. The provider certifies that you have "undergone the necessary sex reassignment procedures required for social gender recognition" and checks male or female. The form does not ask what treatment you had, and surgery is not required. Only M and F are available.
Changing your name on the card is a separate requirement: bring a certified copy of the court order, and update Social Security first. Nebraska law requires you to update your card within 60 days of a legal name change. Both changes can be done at the same visit.
Steps
Have a medical provider sign the DMV form[2]
Print the Certification of Sex Reassignment and ask your MD, DO, PA, APRN, or chiropractor to complete and sign it. Nebraska Medicine's Gender Care Clinic lists help with documents for legal gender marker changes among its services.
If your name is changing, update Social Security first[6]
Bring a certified copy of your name-change decree to Social Security before going to the DMV, so your records match.
This cannot be done online. Bring: the signed certification (for a sex change), a certified court order or other document linking your old and new names (for a name change), one proof of identity with your name and date of birth, your current card, and two proofs of address if your address has not been verified or you moved. Fill out the data form at the office, take a new photo, and pay the fee. In Douglas and Sarpy counties you go to a Driver License Service Center; elsewhere the county treasurer's office handles licensing.
You get a 30-day receipt at the counter. The card is mailed within 14 business days. If it has not arrived in 20 days, call Driver Licensing Services at 402-471-3861. Your old card becomes invalid.
