You can change the gender on your Puerto Rico driver's license or ID to F or M by filing form DTOP-DIS-324 at a CESCO office. Attach a birth certificate or passport that already shows the change, or have any licensed health or behavioral health professional who treats you sign Part B. There is no X option.

How it works
DTOP (Departamento de Transportación y Obras Públicas) processes driver's licenses and ID cards through its CESCO offices (Centros de Servicios al Conductor). Its form DTOP-DIS-324, "Solicitud para el cambio de género de personas transgénero", lets you ask for a license or ID card showing female or male.
You sign a statement under penalty of perjury that the request matches the gender you identify with and is not for fraud. Then you attach one of:
- a birth certificate showing the change,
- a passport showing the change, or
- Part B of the same form, signed by a clinical professional who has a relationship with you: psychologist, therapist, social worker, physician, case manager, or counselor, with their license number.
You do not need surgery or a court order.
Changing your name is a separate part of the same visit. You need your certified court resolution (or a birth certificate showing the new name) and an updated Social Security record.

Bring a birth certificate or passport that already shows your gender, or ask your clinician to complete and sign Part B of DTOP-DIS-324. If your name is also changing, update Social Security first and bring the certified court resolution.

Pay with internal revenue stamps[2]
CESCO fees are paid with Hacienda internal revenue stamps (sellos) and vouchers (comprobantes), which you can buy at collection offices or online. Ask CESCO which amount applies to a gender or name change; the duplicate fees are shown below.
Costs and fee waivers
No fee waiver is available for this step.[2]
We found no general fee waiver for licenses or ID cards on CESCO's page.
| What | Cost | Notes |
|---|---|---|
| Driver's license duplicate[2] | $8 | $6 internal revenue stamp (code 5120) plus $2 voucher (code 0842). Add $17 (code 2028) the first time you validate REAL ID. The CESCO page does not list a separate fee for a gender change. |

After you finishYour license or ID is often the easiest document to change first.
Your license or ID is often the easiest document to change first. With it you can ask the Registro Demográfico to change the gender on your birth certificate (it accepts a license that shows the change). The U.S. passport and Social Security record currently cannot show a changed sex marker; your Puerto Rico card is still valid ID.
Tips3 tips from people who have done this
- A case manager or counselor can sign Part B, as long as they give a license number (theirs or their supervisor's). You do not need a psychiatrist or endocrinologist.
- For REAL ID, CESCO accepts only the Puerto Rico birth certificate version issued since 2010.
- If staff refuse the form, ask for a supervisor and show the DTOP-DIS-324 printed from CESCO's own forms page. The ACLU de Puerto Rico can help if you are turned away.
