How it works
Nevada treats a gender change as a correction under NAC 440.030. You (or a parent, for a minor) sign a notarized Affidavit for Correction of a Record stating the change. Someone else with personal knowledge signs a notarized Supplemental Affidavit. No medical proof or surgery is required, and a court order is optional: if you have one, it replaces both affidavits.
The fee is $45, which includes the correction and one certified copy. Extra copies are $25 each. Processing takes 4 to 6 weeks. A4TE reports that the certificate can show F, M, or X (see review notes).
Name change: for anyone older than one year, a name change on the birth certificate requires a certified court order. Send it with the Court Ordered Change application. You can send both requests together.
This process is only for people born in Nevada. If you were born in another state, follow that state's rules.
Steps
Complete the application and the Affidavit for Correction. Ask someone who knows you to complete the Supplemental Affidavit. If you are also changing your name, add the Court Ordered Change application and a certified copy of your court order.
Get both affidavits notarized[1]
Each person signs in front of a notary. Many banks and UPS stores notarize for free or for a small fee.
Mail everything with ID and payment[1]
Include a copy of the photo ID of the person signing the affidavit, and a $45 check or money order payable to "Office of Vital Records". For credit cards, use the Authorization for Credit Card Use form. Do not send cash. Mail to Office of Vital Records and Statistics, Attn: Corrections, 4150 Technology Way, Suite 104, Carson City, NV 89706. Questions: 775-684-4242 or ovrpac@health.nv.gov.

Application for a Correction of a Birth Record, with Affidavit for Correction of a Record and Supplemental AffidavitSometimes required: Use this if you are changing your sex marker without a court order.
How to fill it out
- Page 1 (application): your contact details. Under "Reason for Correction," write "gender change." Fill in the person-of-record details exactly as on the current certificate.
- Page 2 (Affidavit for Correction): under "Statement of Corrections," list the Sex item, what it says now, and what it should say (F, M, or X). Under "Why are corrections necessary?", a short statement such as "to reflect my gender identity" is enough. Sign in front of a notary.
- Page 3 (Supplemental Affidavit): signed by a different person with personal knowledge, such as a friend, relative, or provider. They describe how they know you. It must also be notarized.
- Use blue or black ink only. Forms with white-out, cross-outs, or blanks are returned.
Application for a Correction of a Record, Court Ordered Change OnlySometimes required: Use this for a name change, or for a sex change if you have a court order.
Use this for a name change, or for a sex change if you have a court order. Include a **certified** copy of the order.

Use your corrected birth certificate for your passport application, if the federal policy allows, and for any agency that asks for proof of sex. Keep the certified copy safe, and order extras now at $25 each.
Tips3 tips from people who have done this
- The supplemental affidavit must come from someone other than you. A friend, partner, family member, therapist, or doctor who knows about your gender identity can sign it.
- No fee waiver is listed. The state does offer a separate free birth certificate application for adults experiencing homelessness, for copies only.
- Under NAC 440.035(5), the registrar needs a court order to change information that was already corrected once. If you might switch between markers later (for example, from F to X), a court order may be needed the second time.
